- Skilled Nursing: wound care, education, observation and assessment, medication administration.
- Physical Therapy: rehabilitation, gait training, strengthening.
- Occupational Therapy: home modifications, adaptive strategies, energy conservation techniques, rehabilitation.
- Speech Therapy: rehabilitation for swallowing, chewing, speech, and cognition - especially after a stroke.
- Home Health Aides: personnel specially trained in health related activities of daily living for homebound patients. Light cleaning in patient areas, patient's laundry, light meal preparation, grooming, bathing, dressing, changing bed sheets, etc.
- Medical Social Work: identifying needs, coordinating community resources for the benefit of our patients, and finding services.
- Nurse Case Managers: coordinating your in-home health care team. Serving as a knowledgable point of contact for patients and their doctors.
- Case Management Assistants: assisting case managers and following the status of patients while home health care is on hold (e.g. patient is not at home because of hospitalization or visit to out-of-town family).
- Home Health Liaisons: trained professionals who represent the home health care option to doctors, facilitate home health referrals, and help coordinate with each patient's case managers at institutions.
